This form will provide the kura with information required for the COVID-19 Student Vaccination Register

Our Kura is now required to gather information about the vaccination status of our tamariki and taiohi aged 5-18 years.
This information will be stored on our kura student database along with all other medical information for our tamariki and taiohi. It could be used to provide general information to the Board of Trustees, the Ministry of Education and the Ministry of Health if requested. General information means we could provide numbers showing how many of our students have received their first vaccination, second vaccination and the number of students who have not been vaccinated. Individual students will not be named in any information the kura shares.
A record of immunisation is kept for all our tamariki entering kura so requesting this information is consistent with our kura enrolment policies. However, as this situation is quite new to us and some whānau may still be deciding their options whānau may choose not to provide the information requested. E pai ana tēnei, this is your choice. Just please understand, if you choose not to respond to this survey or if you choose not to disclose information regarding your child's vaccination status it will be assumed that your child has not been vaccinated.
If whānau prefer to discuss your whānau circumstances face to face then we are happy to organise meetings.
